[ir][spirv-writer] Implement DP4A builtins

Use the SPIR-V builtin polyfill transform to map them to new `sdot`
and `udot` SPIR-V builtins.

De-duplicate extensions in the SPIR-V module and switch capability
deduplication over to a Hashset.
