[ir] Add OperandInstruction class

Use this for all instructions that have operands, putting their
operands in the operand list.

This changes some friendly names getters (such as Call::Args() and
Access::Indices()) to return a utils::Slice instead of a
utils::VectorRef, in order to skip over unrelated operands.
