[ir] Use `Undef` instead of nullptr Swap any use of a `nullptr` operand to create an explicit `Undef` value instead. The IR validator now rejects any `nullptr` operand, and only allows `Undef` for `var` and `terminator` instructions as before. The IR disassembler now emits `nullptr` as `<nullptr>`, to distinguish it from `Undef` which is emitted as `undef`.
